Immunization requirements are mandatory and must be fulfilled before entering a clinical setting. The clinical placement requirements are not subject to accommodations for personal or philosophical reasons and ONLY a medical exemption can be accepted.

As a Health Care Professional, you have an obligation to protect your patients, colleagues and yourself from infections that can be transmitted in health care practice settings.

You are responsible or keeping copies of all your documents, as our office will NOT provide them. The Office of Risk Management is also not responsible for providing proof of immunization to other medical education programs, hospitals or for college licensing.

For your initial submission, all medical and health sciences students need to complete the Student Placement Requirements Record and submit it electronically to the CPRM risk management nurse advisor responsible for your clinical placement file. It must be dated and signed by your physician or other health care professional.
