Immunization

Immunization requirements are mandatory and must be fulfilled before entering a clinical setting. The clinical placement requirements are not subject to accommodations for philosophical reasons and ONLY a medical exemption can be accepted.


As a Health Care Professional, you have an obligation to protect your patients, colleagues and yourself from infections that can be transmitted in health care practice settings.


You are responsible or keeping copies of all your documents, as our office will NOT provide them. The Office of Risk Management is also not responsible for providing proof of immunization to other medical education programs, hospitals or for college licencing.
 

For your initial submission, all health sciences students need to complete the Health Sciences_Clinical Placement Requirements Record Form and all medical students need to complete the Medicine_Clinical Placement Immunization and Consent Form and submit it electronically to the person responsible for their clinical placement file from the CPRM team. It must be dated and signed by your physician or other health care professional.
